Hospice Eligibility Criteria
Beaumont’s Trusted Hospice Care
Skilled nurses, certified nursing aides, chaplains, counselors and volunteers work together to help manage pain, ease symptoms and assist with everyday activities. Three Oaks Hospice also offers bereavement support for families, and hospice services are available in your home, in a relative’s home, or wherever you call home, so you can remain in a comfortable, familiar environment.
Services Provided
Three Oaks Hospice develops an individualized care plan for each patient. We can assist with the following services in Beaumont:
- Medical equipment & supplies
- Related medications
- Pain management and symptom control
- Physician directed services
- Nursing care
- Chaplain services
- Social work and counseling services
- Volunteer support
- Bereavement support
- Other therapies as needed
