Three Oaks Hospice’s skilled nurses, certified nursing aides, chaplains, counselors, and volunteers work together to help manage pain, ease symptoms, and assist with everyday activities. Three Oaks Hospice also offers bereavement support for families, and hospice services are available in your home, in a relative’s home, or at assisted living facilities, so you or your loved one can remain in a comfortable, familiar environment.

We Honor Veterans Program in San Antonio

Three Oaks Hospice | San Antonio has reached level 2 with the We Honor Veterans program. This achievement results from our commitment to establishing organizational policies that include a military background checklist during admission. We ensure that our staff and volunteers are knowledgeable about providing care specific to Veterans. Additionally, we offer resources to the community through our Hospice-Veteran Partnership.

Partnering with hospices, veterans’ centers, and veterans directly, the We Honor Veterans program is a key resource associated with the National Hospice and Palliative Care Organization (NHPCO). The We Honor Veterans Program offers varying partnership levels based on specific prerequisites, from Recruit to Level 5. By joining forces with them, Three Oaks Hospice guarantees that our Veterans are treated with the utmost respect, dignity, and individualized care.

Hospice Medical Director

The hospice medical director helps deliver and manage care while working with the patient’s current physicians.

Hospice Nurses

Hospice nurses give health assessments, set up medical equipment, manage and schedule medications, and communicate with the patients and families about any health issues or emotional concerns.

Hospice Aides

Hospice aides provide companionship and emotional support to patients who might otherwise experience loneliness during this journey. They help the patient with personal hygiene and bathing, hoping to make the patient as comfortable as possible toward the end of life.

Social Workers

Social workers offer comfort and support for the family in order to reduce stress. They also assist with getting the family in contact with various local programs their community has to offer. Social workers help manage legal and financial paperwork as well.

Hospice Volunteers

Hospice volunteers give their time to serve our patients and their families. They can do this in many ways. Volunteers can help run errands for the family, provide companionship, read a book, play an instrument, or help with household tasks.

Spiritual Advisers

Spiritual advisers can offer support to families who are interested in talking about spiritual concerns associated with the end of life. They are able to bring a sense of peace to families during a difficult time.
