The Hospice Care Team

The Three Oaks Hospice Houston team is made up of various professionals and volunteers, all working toward the same goal: keeping the patient comfortable during the end stages of life. Here are the members of the hospice care team:

Hospice Nurses

Hospice nurses come to the patient’s home to monitor patient health, perform basic medical tests and help family members better understand how to care for patient needs.

Hospice Volunteers

Hospice volunteers help ease patient loneliness with regular visits. The volunteers on our team might read a patient’s favorite book or engage in conversation about an individual’s favorite topics.

Hospice Aides

Hospice aides serve as companionship for a hospice patient while also helping with everyday personal tasks such as personal grooming and bathing.

Social Workers

Social workers manage the practical aspects of hospice care, including helping with financial or legal matters and arranging access to community resources.

Spiritual Advisers

A chaplain on our hospice care team can offer support and advice on spiritual issues to hospice patients with concerns or curiosity about end-of-life issues.

Bereavement Counselors

Bereavement counselors provide support for family members after a loved one’s death, including grief counseling and assistance with funeral arrangements.

Palliative Care in North Houston
