A woman meeting with a hospice doctor to discuss quality of life and supportive care options

Primary Care is offered in Utah and Arizona and functions as a mobile doctor’s office. The same services and coordination of care you receive at a traditional doctor’s office are available in the comfort of your group home or community

Mobile Primary Care is a healthcare service that brings primary care physicians directly to patients who may have difficulty traveling to a doctor’s office due to chronic illness or other health issues. Home health care services are especially important for those who need frequent medical attention but may experience discomfort or pain while traveling. By providing on-site medical care, we ensure patients receive vital healthcare when they need it, and without having to leave the comfort of their own homes.

Frequently Asked Questions About Primary Care

Answers to common questions regarding home health care services can be found here.

Who Provides Primary Care?

Our primary care team includes a specialized group of nurse practitioners who manage and coordinate care across all specialists as needed, based on your care needs.  Coordination may include other doctors, therapists, other health care professionals.

How Does Mobile Primary Care Differ from the Care I’m Getting Now?

Most patients see their physicians in a traditional clinic setting where the patient comes to the doctor. However, since we all feel most comfortable at home, why not have our primary care nurse practitioners come into your home? That is exactly what you get when you receive primary care from us— qualified, professional primary care provided directly within the comfort of your care facility or community.

Comprehensive, patient-centered care is paramount for individuals with advanced illnesses. Our team  exemplifies this approach by offering a continuum of care that spans from mobile primary care to palliative care and, ultimately, to hospice care. This continuum ensures patients receive appropriate care at every stage of their illness, improving their quality of life and providing support to their families throughout.

How Do I Know If I Need Primary Care?

Far too often, people let pain or anxiety stop them from seeking care from their primary care provider. This problem can be even worse for those facing severe, chronic illness. And it is for this purpose we bring the primary care directly to the patient’s home. If you or your loved one have difficulty going to see your doctor because of pain caused by severe illness, we want to provide primary care for them within the walls of their home.

Where Can I Have Primary Care?

Primary care can be provided in assisted living facilities, nursing homes, outpatient palliative care clinics, and other specialized clinics or in the comfort of your own home.

Will My Insurance Cover Primary Care?

Most private insurance plans cover some or all primary care services. Medicare and Medicaid also typically cover most of these services. Many insurance providers do require a small co-pay for primary care services. Veterans may also be eligible for primary care through the Department of Veterans Affairs.

How Does Primary Care Differ from Receiving Care at a Doctor’s Office?

Why travel to the doctor’s office when you can have quality primary care delivered right at home? Mobile primary care provides many of the services traditionally offered at a doctor’s office right within the comfort of the patient’s home. Our providers coordinate with other mobile services to provide additional support such as mobile X-rays, Ultrasounds, lab work and many more services in the place they feel most comfortable.

What Are the Benefits of Using Primary Care?

Studies have shown that in-home primary care can have many benefits for both patients and their families. These studies show that those enrolled in in-home primary care have fewer symptoms, greater emotional support, and increased patient and family satisfaction.

Can My Primary Care Physician Stay Involved in My Care?

No, we assume all services as primary care, but patients can continue to see their specialists as needed.

Will I Have Any Out-of-Pocket Expenses for Primary Care?

The cost to receive Primary Care services is similar to the costs the patient would face when visiting their doctor. Primary Care visits commonly require a small co-pay charge depending on the patient’s insurance. You will have full knowledge of all the financial implications of Primary Care prior to admission.
