Three Oaks Hospice delivers compassionate end-of-life care to patients in the Chicago area. Hospice care focuses on symptom management and quality of life for individuals with a life-limiting illness.

During hospice care, a skilled team of nurses, therapists, social workers, clergy, and volunteers works together to bring peace and comfort to patients facing the end of life. Pain management, chaplain services, counseling, and medical equipment management are all part of hospice care. Physician-directed care tailored to each patient’s individual needs and preferences lets you or your loved one spend those final days, weeks, or months in a familiar setting surrounded by love and support.
