Introduction:

At Three Oaks Hospice, we are committed to addressing the spiritual needs of our patients, families, caregivers and staff. As a Hospice Chaplain, you will be pivotal in providing pastoral care interventions, including pastoral counseling, tailored to individual spiritual assessments based on their values, beliefs and religious orientations.

Job Responsibilities:

  • Spiritual Support and Counseling: Offer spiritual support to patients, families, and bereavement assistance following the passing of a loved one. This includes going beyond traditional prayer to meet the unique spiritual needs of each individual.
  • Spiritual Assessments: Conduct thorough assessments of hospice patients and their support systems to identify spiritual needs, utilizing tools such as Plans of Care (POC), clinical records and Interdisciplinary Group (IDG) conference minutes.
  • Interdisciplinary Collaboration: Actively participate in interdisciplinary team meetings and engage in the patient care planning process. Your input will be crucial in creating comprehensive care plans that address spiritual needs.
  • Quality Improvement: Participate in the Quality Assessment Performance Improvement (QAPI) process as required, contributing to the overall enhancement of hospice care services.
  • Communication and Teamwork: Utilize and promote effective interpersonal relations and communication skills. Facilitate using these skills among other team members to achieve desirable outcomes and foster a supportive environment.
  • Cultural and Religious Sensitivity: Honor each patient’s spiritual beliefs and respect family cultural traditions. Work closely with the patient’s clergy when requested, ensuring a respectful and inclusive approach to spiritual care.

Applicant Requirements / Qualifications:

  • Educational Background: A degree in divinity, theology, spiritual care or a related field is preferred.
  • Certification: Certification or eligibility for certification as a chaplain by a recognized chaplaincy certifying body.
  • Experience: Prior experience in a hospice, hospital or similar environment is highly desirable.
  • Interpersonal Skills: Strong ability to communicate effectively and compassionately with patients, families and team members.
  • Cultural Competence: Demonstrated ability to work respectfully with diverse cultural and religious backgrounds.
