Hospice Care

From the start, our hospice care focuses on symptom management and quality of life for individuals facing terminal illnesses. A team of nurses, social workers, hospice aides, volunteers, and others work together to manage pain, provide companionship, offer comfort, and ease family caregivers’ burden. We manage pain and other symptoms, offer companionship, relieve caregiver burden and allow patients to remain in the comfort of home, a loved one’s home or a medical setting.

Palliative Care

Palliative Care

Our Palliative care program is designed for individuals living with serious illness who are still receiving curative or life-prolonging treatments. This type of care focuses on relieving symptoms such as pain, fatigue, nausea and shortness of breath, while also addressing emotional, social, and spiritual concerns. 

Our interdisciplinary team coordinates closely with your existing medical providers to ensure care is seamless and aligned with your goals. Services can be provided at home or in a facility, and evolve as your needs change.

Primary Care

Our primary care coordination out of Arizona & Utah with supports individuals who require regular, ongoing management of chronic or complex health conditions. Our experienced care team works alongside your doctors and specialists to streamline communication, oversee medication adherence and monitor changes in condition. 

Whether you are transitioning from hospital to home, living with multiple diagnoses, or planning for future needs, our approach ensures continuity of care, peace of mind, and a higher quality of life for both patients and caregivers.

Hospice Eligibility Criteria

Neurological Disorders

End-of-life care for conditions including dementia, Parkinson’s, ALS, MS, or other progressive disorders.

Oncological Diseases

When curative treatment for cancer is no longer effective and the focus shifts to comfort-based care.

Cardiovascular Diseases

End-of-life care for CHF, stroke, or other cardiac conditions requiring intensive symptom management.

Pulmonary Disease

Hospice for severe COPD, pulmonary fibrosis or respiratory failure where quality of life is deeply impacted.

Renal Disease

End-stage kidney disease when dialysis is no longer effective, pursued or is unsuitable for the patient.

Liver disease

Hepatic Disease

Advanced hepatic failure or end stage liver disease where symptom relief and comfort are the central focus
