Job Responsibilities:

  • Personal Healthcare Services: Provide personal healthcare and related services to patients in their residences, ensuring their comfort and safety.
  • Safe Environment Maintenance: Assist in maintaining a safe and clean environment, working cooperatively with patients and families.
  • Observation and Reporting: Share observations and problems with the supervising registered nurse, contributing to the overall care plan.
  • Direct Patient Care: Provide direct patient care under the direction of the RN, adhering to the Aide Plan of Care.
  • Patient Assistance: Following agency policies, utilize skills to assist patients with safe transfers and ambulation.
  • Documentation and Reporting: Report changes in patient conditions and document care rendered, ensuring continuity of care.
  • Communication: Promote positive, supportive, and respectful communication with patients, families, and fellow employees.

  • Respectful Environment: Foster an environment that respects patient privacy and property.

Applicant Requirements / Qualifications:

  • Graduate of an accredited Associate or bachelor’s degree in Nursing
  • Current state license as an RN and current state driver’s license
  • Two years experience as an RN in a clinical care setting; hospice or home health experience preferred
  • Current CPR certification
  • Proficient skill using a computer is required; Microsoft Office suite experience is preferred
  • Strong interpersonal skills and ability to provide effective support to the patient and family
  • Demonstrate skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles
