The primary function of this role is to provide coordinated care in patients’ homes across all age groups. This includes performing psychosocial assessments, analysis, counseling, and referrals to meet the diverse needs of patients and their families. Job Responsibilities:

  • Patient and Family Support: Assist patients and families in navigating medical paperwork and making sensitive healthcare decisions.
  • Community Resource Provision: Provide access to and information about community resources relevant to patient and family needs.
  • Emotional Support: Offer compassionate emotional support to patients and their families, helping them cope with end-of-life challenges.
  • Psychosocial Assessments: Conduct thorough psychosocial assessments to understand and address the needs of patients and families.
  • Counseling Services: Provide counseling services tailored to the individual needs of patients and families.
  • Referral Services: Make appropriate referrals to other healthcare professionals or community services as needed.
  • Team Collaboration: Work closely with healthcare professionals passionate about patient-centered care.
  • Flexible Scheduling: Utilize the flexibility offered to create a schedule that best meets the needs of your patients.
  • Leadership and Learning: Engage in opportunities to listen, learn and grow as a leader within the hospice care team.

Applicant Requirements / Qualifications:

  • Bachelor’s or master’s degree in social work from a state-accredited program.
  • One (1) year of social worker experience in a health care agency.
  • Working knowledge of community resources. Good interpersonal skills.

  • Current State Certification.

  • Reliable transportation with a valid license & insurance.
